Q: Are telehealth services available?
A: Yes, our practice offers telehealth services for follow-up appointments. Initial assessments should be completed in-person at our office location in order to provide the best care possible.
Q: Do you accept health insurance?
A: We currently work with CDPHP Commercial lines only. Otherwise, we accept private pay (rates $300 for your initial appointment & $150 for follow-ups). We provide Super bills which can be submitted to your insurance for out-of-network reimbursement – please contact your insurance to see what reimbursement you may qualify for. Your insurance reimbursement is not guaranteed by Iadeluca NP Psychiatry.
Q: How do I make an appointment?
A: Please visit our homepage http://www.iadelucapsychnp.com and scroll to the bottom. Click “Request an appointment” to request an appointment.
Q: What methods of payment are accepted?
A: Checks, cash and all major credit cards are accepted (Visa, Mastercard, Discover and American Express). Once your card is on file, payments will be easily fulfilled at time of service.
Q: Where is your office located?
A: Our office is conveniently located at 7 Corporate Drive, Clifton Park, NY 12065. *Please Note: our office is shared with “With Grace Mental Health Counseling PLLC.”
Q: What forms are required to be completed prior to my initial consultation?
A: Once your initial appointment is accepted, you will be invited to create your client portal. Forms will be sent electronically for you to complete prior to your initial assessment. This is to provide the most thorough care possible.
