Forms

Once your appointment has been confirmed, patient intake forms can be completed electronically via the patient portal: phr2.charmtracker.com

Please complete and submit these forms electronically through the portal at least 48 hours before first appointment. Forms take about 15 to 20 minutes to complete. If your forms are not completed before your appointment, the appointment will be cancelled, and you will need to reschedule.
